It is now unused. We cannot yet remove the streamout functionality in u_blitter
as r600g still uses it for clear_buffer on GPUs older than Evergreen.

This wires up the shader in the previous commit so we can blit without
having to reimplement the blitter in the D3D12 driver.
This isn't optimal, but it's reasonable enough, and this behavior is
rare.

This is for drivers that can't map depth and stencil and need to blit
them to a color texture for CPU access.
This also useful for drivers using separate depth and stencil.

For V3D, the HW will interpolate slightly differently along the shared
edge of the trifan. The conformance tests manage to catch this in the
nearest_consistency_* group. To get interpolation to match, we need the
last vertex of the triangle to be shared.
I first tried implementing draw_rectangle to do triangles instead, but
that was quite a bit (147 lines) of code duplication from u_blitter, and
this seems much simpler and less likely to break as u_blitter changes.
Fixes dEQP-GLES3.functional.fbo.blit.rect.nearest_consistency_* on V3D.

Like the r600 paths to use other custom states, we pass in a couple of
parameters to customize the innards of the blitter. It's up to the caller
to wrap other state necessary for its shaders (for example, constant
buffers for the uniforms the shader uses).

All bound layers (from first_layer to last_layer) should be cleared.
This uses a vertex shader which outputs gl_Layer = gl_InstanceID, so each
instance goes to a different layer. By rendering a quad and setting
the instance count to the number of layers, it will trivially clear all
layers.
This requires AMD_vertex_shader_layer (or PIPE_CAP_TGSI_VS_LAYER), which only
radeonsi supports at the moment. r600 could do this too. Standard DX11
hardware will have to use a geometry shader though, which has higher overhead.
For conditional rendering this makes it possible to skip rendering
if either the predicate is true or false, as supported by d3d10
(in fact previously it was sort of implied skip rendering if predicate
is false for occlusion predicate, and true for so_overflow predicate).
There's no cap bit for this as presumably all drivers could do it trivially
(but this patch does not implement it for the drivers using true
hw predicates, nvxx, r600, radeonsi, no change is expected for OpenGL
functionality).
