[dxgi] unchain DxgiFactory::CreateSwapChain and CreateSwapChainForHwnd 

similar to https://github.com/doitsujin/dxvk/pull/3966, avoid
chaining so that dxgi tools attempting to wrap swapchains don't
end up double wrapping
