Previously this check was skipped for pools with
VK_DESCRIPTOR_POOL_CREATE_FREE_DESCRIPTOR_SET_BIT unset, but after
96a240e1 we need to check this otherwise we risk overflowing
radv_descriptor_pool::entries into the host memory base
This fixes a crash to desktop when launching Dota 2, which overallocates
descriptor sets and expects an error to allocate another descriptor pool

With VK_EXT_graphics_pipeline_library, pipeline layouts created with
VK_PIPELINE_LAYOUT_CREATE_INDEPENDENT_SETS_BIT_EXT might contain NULL
descriptor sets.

The spec states that descriptor set layouts can be destroyed almost
at any time:
"VkDescriptorSetLayout objects may be accessed by commands that
operate on descriptor sets allocated using that layout, and those
descriptor sets must not be updated with vkUpdateDescriptorSets
after the descriptor set layout has been destroyed. Otherwise,
descriptor set layouts can be destroyed any time they are not in
use by an API command."

Uplay needs this to avoid a crash because it does an use-after-free
of a descriptor set layout. This was initially introduced by Bas to
workaround a similar issue with Baldur's Gate 3, it seems needed again.

"This parameter is ignored if templateType is not
VK_DESCRIPTOR_UPDATE_TEMPLATE_TYPE_DESCRIPTOR_SET."
This fixes an assertion about the base object type when running Yuzu
with Vulkan validation layers enabled.

The VK_ERROR_FRAGMENTED_POOL and VK_ERROR_OUT_OF_POOL_MEMORY errors are
not as exceptional cases as most. These are expected to be hit by
applications in the normal course of doing their thing. Probably best
not to spam stderr and the debug logs with them.

We perform a FMASK expand when transitioning to GENERAL or TRANSFER_DST
layout, so storage images always have an identity FMASK.
radeonsi doesn't appear to expand the FMASK for read-only storage images,
so the sample index adjustment is still needed there.

The Vulkan specification says: "If VkDescriptorSetAllocateInfo::pSetLayouts[i]
does not include a variable count descriptor binding, then
pDescriptorCounts[i] is ignored". The previous code triggered an assertion
in such cases, and this patch fixes it.
v2: removed the offending assertion that is now always satisfied and
reworded the commit message with a reference to Vulkan spec.

Allocating a descriptor set is aligned to 32 bytes, so just like the
other buffer types, bump the descriptor size to 32 bytes when allocating
MUTABLE descriptor types from a pool.

MAX2(count * struct size, 1) results in 1 for count=0, not the size of a struct.
Since this MAX only seems to exist so we can keep using NULL for error reporting,
just refactor to return a VkResult.
